Endovenous laser or radiofrequency treatment of varicose veins

Varicose veins: laser and radiofrequency in Tunis, Tunisia · Centre Urbain Nord

Closing the varicose vein from the inside using heat from a laser fibre or radiofrequency catheter, without an incision.

What is it?

Endovenous treatment is a modern alternative to stripping for varicose veins caused by a faulty saphenous vein. Instead of removing the vein, it is closed from the inside: it gradually turns into a fibrous cord that the body absorbs. Blood then flows through the healthy veins. The doctor punctures the vein under ultrasound guidance, usually at the knee or calf, and slides a laser fibre or radiofrequency catheter up to the top of the thigh. After injecting a numbing fluid around the vein (tumescent anaesthesia), the fibre is slowly withdrawn while heating the vein wall, which seals the vein. Visible varicose branches can be removed at the same time through small phlebectomies. This technique is suitable when the saphenous vein is fairly straight and of the right size, as shown by duplex ultrasound. It leaves few marks, causes less bruising than stripping and allows a quick return to normal activities. The choice between laser, radiofrequency and surgery is made with the surgeon according to the anatomy of the veins. At Clinique Pasteur Tunis, this treatment is carried out as day surgery, under ultrasound guidance, most often under local anaesthesia, with discharge home the same day.

When is it indicated?

Varicose veins due to a leaking saphenous vein

When duplex ultrasound shows that the saphenous vein no longer carries blood upwards, closing it removes the cause of the varicose veins.

Heavy legs, pain, swelling

Symptoms caused by venous pooling are usually relieved once the faulty vein is closed.

Skin changes or venous ulcer

Treatment helps protect the skin and supports healing of an ulcer.

Wish for a quick recovery

Usually with no incision or general anaesthesia, walking is immediate and normal activities resume quickly.

Preparation

1Venous duplex ultrasound to confirm the vein is suitable for the technique
2Prior consultation with the surgeon and, if needed, the anaesthetist
3Buying the prescribed compression stockings
4Reporting current medication, especially anticoagulants
5Arriving with a clean leg, no cream, and someone to accompany you home

Procedure

1

Settling in the procedure room and locating the vein on ultrasound

2

Puncture of the vein and insertion of the fibre or catheter

3

Injection of anaesthetic around the vein under ultrasound guidance

4

Gradual withdrawal of the fibre while heating the vein wall

5

Additional phlebectomies of visible branches if needed

6

Dressing, compression and getting up straight away

Duration

30 minutes to 1 hour

Aftercare & Recovery

Patients go home the same day. Walking is encouraged from discharge. A pulling sensation along the thigh is common during the first week. Most patients resume their activities within 2 to 3 days. Compression stockings are worn for a few weeks and a follow-up duplex scan checks that the vein is closed.

Risks and side effects

Bruising and a tight cord-like feeling along the treated vein, temporary
Brownish skin discolouration along the vein, which usually fades
Localised skin numbness, usually temporary
Thrombosis or clot extension into the deep vein, rare, checked at follow-up
Reopening of the vein or new varicose veins over time

These risks remain rare. Your surgeon will inform you in detail before the procedure.
