Varicose vein surgery (stripping and phlebectomy)

Varicose veins: surgery in Tunis, Tunisia · Centre Urbain Nord

Surgical removal of varicose leg veins by stripping the saphenous vein and through tiny incisions (phlebectomies).

What is it?

Varicose veins are dilated, twisted superficial leg veins whose valves no longer work properly: blood pools instead of flowing back up to the heart. They cause heavy legs, swelling, cramps and, over time, skin changes or even ulcers. Conventional surgery often combines two procedures. Stripping removes the faulty saphenous vein, through a small incision in the groin or behind the knee, using a guide wire passed along the vein. Phlebectomies remove the varicose branches through tiny incisions a few millimetres long, which often need no stitches. Treatment is offered when varicose veins are troublesome or complicated, after a duplex ultrasound has shown exactly which veins are affected. Depending on the case, the surgeon may instead suggest an endovenous technique (laser or radiofrequency) that closes the vein without removing it. The healthy deep veins then carry the blood back. At Clinique Pasteur Tunis, varicose vein surgery is performed in the operating theatre, most often as day surgery or with one night in hospital, with compression stockings and a quick return to walking.

When is it indicated?

Troublesome varicose veins

Heavy legs, pain, swelling at the end of the day or night cramps can be relieved by removing the faulty veins.

Skin changes on the leg

Brown discolouration, eczema or hardening around the ankle show that venous pooling is damaging the skin: treating the veins protects it.

Venous ulcer or past ulcer

Stopping the backflow in the faulty veins helps the ulcer heal and lowers the risk of it coming back.

Inflamed or bleeding varicose vein

A varicose vein that has become inflamed (superficial thrombophlebitis) or has bled warrants treatment.

Veins too wide or tortuous for an endovenous technique

In these cases, surgical removal remains the most suitable solution.

Preparation

1Venous duplex ultrasound mapping the veins to be treated
2Anaesthetic consultation
3Marking of the varicose veins with a pen, standing up, the day before or the morning of surgery
4Buying the prescribed compression stockings and bringing them on the day
5Antiseptic shower and skin preparation as instructed
6Fasting for 6 hours before the operation

Procedure

1

General, spinal or local anaesthesia depending on the extent of the veins

2

Small incision in the groin or behind the knee

3

Tying off the saphenous vein where it joins the deep vein

4

Removal of the saphenous vein using a guide wire (stripping)

5

Removal of varicose branches through micro-incisions (phlebectomies)

6

Dressing and compression applied before waking

Duration

45 minutes to 1.5 hours depending on the number of veins treated

Aftercare & Recovery

Walking resumes the same day. Patients go home the same day or the next day. Bruising and tenderness along the leg fade within 2 to 3 weeks. Compression stockings are worn for a few weeks. Return to work takes 1 to 3 weeks depending on the job. A check-up appointment is planned; new varicose veins may appear over the years.

Risks and side effects

Bruising and small haematomas along the leg, common and temporary
Patch of numb skin, which usually improves within a few months
Lymphatic leakage or wound infection, rare
Deep vein thrombosis, rare, prevented by early walking and treatment if needed
New varicose veins appearing over time

These risks remain rare. Your surgeon will inform you in detail before the procedure.
