Abdominal aortic aneurysm surgery (open repair)

Aortic aneurysm, open repair in Tunis, Tunisia · Centre Urbain Nord

Replacement of the dilated segment of the abdominal aorta with a graft, through an open approach, to prevent rupture.

What is it?

The aorta is the largest artery in the body. In the abdomen, its wall can weaken and gradually widen: this is an aneurysm. It usually causes no symptoms and is often discovered on an ultrasound or CT scan. The danger is rupture, a serious emergency, and the risk increases with the size of the aneurysm. In open surgery, the surgeon reaches the aorta through an incision in the abdomen or flank, temporarily stops the blood flow, opens the aneurysm and replaces it with a synthetic fabric tube (graft), straight or Y-shaped when the iliac arteries are also affected. The aneurysm wall is then closed around the graft to protect it. Surgery is offered when the aneurysm reaches a size at which the risk of rupture exceeds the risk of the operation (most often around 5 to 5.5 cm), when it grows quickly or when it causes pain. The open approach is chosen in particular when the shape of the aorta is not suitable for a stent graft, or in younger patients, because it gives a durable result with lighter follow-up. The endovascular alternative is always discussed. At Clinique Pasteur Tunis, this major operation is performed in the operating theatre, with post-operative monitoring in intensive care and then on the ward, by a team experienced in both open and endovascular approaches.

When is it indicated?

Large aneurysm

Beyond a certain diameter, the risk of rupture becomes greater than the risk of surgery: replacing it then becomes the safest option.

Rapidly growing aneurysm

Fast growth between two scans is a sign of weakness and leads to surgery without delay.

Painful aneurysm

Abdominal or back pain related to the aneurysm may herald a rupture and requires prompt care.

Aortic shape unsuitable for a stent graft

When the anatomy does not allow a graft to be placed from inside, open surgery remains the standard solution.

Younger patient in good general health

Open repair offers a very durable result and requires fewer follow-up scans in the long term.

Preparation

1CT angiogram of the aorta to measure the aneurysm and plan the operation
2Thorough cardiac and respiratory assessment
3Blood tests including kidney function, and blood group
4Anaesthetic consultation and adjustment of medication
5Stopping smoking and breathing physiotherapy recommended before surgery
6Antiseptic shower, fasting as instructed

Procedure

1

General anaesthesia, often combined with epidural pain relief

2

Incision in the abdomen or flank and exposure of the aorta

3

Temporary interruption of blood flow above and below the aneurysm

4

Opening of the aneurysm and placement of the graft

5

Gradual restoration of blood flow to the legs

6

Closure of the aneurysm wall over the graft, then of the abdominal wall

7

Monitoring in intensive care for the first days

Duration

About 3 to 4 hours

Aftercare & Recovery

After 1 to 2 days in intensive care, the total hospital stay is usually 7 to 10 days. Eating resumes gradually. Recovery takes 6 to 8 weeks, without heavy lifting. Once healed, the graft is generally very durable; a follow-up scan is scheduled later on, together with management of cardiovascular risk factors.

Risks and side effects

Bleeding, compensated by transfusion if needed
Heart or breathing complications, anticipated by the assessment and intensive care monitoring
Temporary impairment of kidney function
Slow return of bowel function in the first days
Incisional hernia at the scar, later on

These risks remain rare. Your surgeon will inform you in detail before the procedure.
