Declotting and revision of an arteriovenous fistula
Fistula revision in Tunis, Tunisia · Centre Urbain Nord
Procedures to unblock or repair a thrombosed, narrowed or complicated dialysis fistula in order to preserve the access.
What is it?

When is it indicated?
If the fistula no longer buzzes under the fingers, it has probably thrombosed: contact the team quickly to have it cleared.
Abnormal pressures or less effective dialysis prompt a search for a narrowing, which can be dilated before the fistula blocks.
This may indicate a narrowing downstream of the needling site, which should be investigated and treated.
Marked widening with thin skin over it carries a risk of bleeding: repair is then offered.
When the fistula "steals" blood from the hand or tires the heart, its flow can be reduced surgically.
Preparation
Procedure
Local anaesthesia or arm block
Small incision over the fistula or puncture for an endovascular approach
Removal of the clot with a balloon catheter if the fistula is blocked
X-ray check of the whole fistula
Correction of the narrowing: balloon, new connection or short bypass
Checking the thrill and closure
Duration
1 to 2 hours depending on the procedure
Aftercare & Recovery
The procedure is usually done as day surgery or with a short stay. The fistula can often be used quickly, sometimes from the next session, depending on what was done. Regular monitoring of the fistula by the dialysis team and by duplex ultrasound helps detect a new narrowing early.
Risks and side effects
These risks remain rare. Your surgeon will inform you in detail before the procedure.
