Total thyroidectomy
Total thyroidectomy in Tunis, Tunisia · Centre Urbain Nord
Complete removal of the thyroid gland for goitre, Graves' disease or a suspicious nodule, followed by lifelong hormone treatment.
What is it?

When is it indicated?
When the thyroid is enlarged with nodules on both sides, removing the whole gland avoids the need for another operation later.
A very large goitre can make breathing or swallowing difficult or extend behind the breastbone: removing it relieves the pressure.
When medication does not control the overactive thyroid, or it keeps coming back, surgery offers a definitive treatment.
When a needle biopsy of a nodule suggests or confirms cancer, removing the thyroid is the first step of treatment.
Preparation
Procedure
General anaesthesia, with the head tilted slightly back
Discreet horizontal incision at the base of the neck
Identification and preservation of the recurrent laryngeal nerves
Identification and preservation of the parathyroid glands
Removal of both lobes and the isthmus of the thyroid
Cosmetic closure, sometimes with a small drain
Monitoring in the recovery room, then on the ward
Duration
1.5 to 2.5 hours
Aftercare & Recovery
The hospital stay is usually 1 to 3 days, with a blood calcium check. Hormone replacement is started quickly and then adjusted by the endocrinologist. The voice may tire easily for a few days. Normal activities can be resumed within 1 to 2 weeks. The scar fades gradually over the months; the results of the gland analysis are discussed at a follow-up appointment.
Risks and side effects
These risks remain rare. Your surgeon will inform you in detail before the procedure.
