Incisional and large hernia repair with mesh

Incisional hernia in Tunis, Tunisia · Centre Urbain Nord

Repair of the abdominal wall through an old scar or of a large hernia, reinforced with a mesh.

What is it?

An incisional hernia forms in the scar of a previous abdominal operation: the wall has not healed properly and lets part of the abdominal contents bulge under the skin. It shows as a swelling that increases when standing and on exertion. Excess weight, smoking, diabetes or a wound infection at the time of the first operation make it more likely. Some very wide umbilical or epigastric hernias are treated in the same way. The repair consists of returning the contents, bringing the muscles back together and reinforcing the wall with a mesh. Depending on size and location, the mesh is placed behind the muscles through an open approach, or inside the abdomen by laparoscopy. For very wide incisional hernias, muscle release techniques make it possible to close the wall without tension. Surgery is offered when the incisional hernia is bothersome, painful, growing, damaging the skin or has already caused episodes of obstruction. For large hernias, it is often useful to prepare for surgery: weight loss, stopping smoking, good diabetes control. At Clinique Pasteur Tunis, incisional hernia repair is planned in consultation with a CT scan of the abdominal wall, which measures the size of the defect and guides the choice of technique. The length of stay depends on the extent of the repair.

When is it indicated?

Bulge in an old abdominal scar

A swelling that appears in a surgical scar, especially when standing or coughing, is an incisional hernia that will not close on its own.

Discomfort, pain or increasing size

When the hernia restricts movement, hurts or grows, repairing it improves comfort and support of the abdomen.

Large umbilical or epigastric hernia

Hernias with a wide opening are repaired like an incisional hernia, with a mesh for a lasting result.

Episodes of obstruction or fragile skin

If the hernia has already become stuck, or the overlying skin is deteriorating, surgery becomes more urgent.

Recurrence after a previous repair

A hernia that comes back can be repaired again, often with a different technique from the first one.

Preparation

1CT scan of the abdominal wall to measure the hernia
2Blood tests and anaesthetic consultation
3Stopping smoking at least one month before, strongly recommended
4Weight loss and diabetes control if needed
5Report any anticoagulant treatment
6Fast for 6 hours before surgery

Procedure

1

General anaesthesia

2

Incision along the old scar, or small incisions away from it for laparoscopy

3

Release of adhesions and return of the contents

4

Bringing together the muscles of the abdominal wall

5

Placement and fixation of a reinforcing mesh

6

Drains if needed, closure and fitting of an abdominal binder

Duration

1 hour 30 minutes to 3 hours depending on size

Aftercare & Recovery

The hospital stay usually lasts 2 to 5 days. An abdominal binder is often worn for a few weeks. Daily activities resume gradually; lifting and sport are avoided for about six weeks. Follow-up consultations monitor healing.

Risks and side effects

Abdominal wall pain for a few weeks, treated with painkillers
Fluid collection under the skin (seroma), which may need to be drained with a needle
Infection of the wound or, more rarely, of the mesh
Possible recurrence, more likely when the hernia was wide
Rarely, injury to the bowel while releasing adhesions, recognised and repaired

These risks remain rare. Your surgeon will inform you in detail before the procedure.
