Inguinal hernia repair (adults)

Inguinal hernia in Tunis, Tunisia · Centre Urbain Nord

Repair of a groin hernia in adults, by laparoscopy or open surgery, usually reinforced with a mesh.

What is it?

An inguinal hernia is a bulge that appears in the groin, where the abdomen meets the thigh. It occurs when part of the abdominal contents (fat or a loop of bowel) pushes through a weak spot in the muscle wall. It is far more common in men, may affect one or both sides, and never heals on its own. There are two main approaches. In open repair, an incision of a few centimetres in the groin crease allows the surgeon to push the hernia back and reinforce the wall with a mesh (a soft, well-tolerated net). In laparoscopic repair, the surgeon works through three small incisions and places the mesh behind the muscles; this approach is often chosen for hernias on both sides or for recurrences. The choice depends on the hernia, age, activity and medical history. Surgery is offered when the hernia is bothersome, painful, getting bigger, or in an active person. Above all, it prevents strangulation, a rare but serious complication in which the hernia becomes trapped. In an older person with a small hernia causing no discomfort, simple monitoring may sometimes be discussed. At Clinique Pasteur Tunis, inguinal hernia repair is usually performed as a short stay or even as day surgery. The surgeon explains the chosen technique during the consultation, and clear instructions on returning to physical effort are given at discharge.

When is it indicated?

Bothersome or painful groin hernia

A groin bulge that pulls, aches at the end of the day or hurts on exertion is worth repairing: the discomfort usually goes away after surgery.

Hernia that is getting bigger

Hernias tend to grow over time; the smaller the hernia at the time of repair, the simpler the operation.

Hernias on both sides

When both groins are affected, laparoscopy often allows both sides to be repaired in the same operation, through the same small incisions.

Recurrence after a previous repair

If a hernia comes back after surgery, a different approach from the first one is generally used, through tissue that has not been scarred.

Preventing strangulation

Repairing a hernia as a planned operation avoids the risk of it becoming trapped one day and requiring emergency surgery.

Preparation

1Consultation with the surgeon and examination of the groin, standing and lying down
2Ultrasound only if the diagnosis is uncertain
3Blood tests and anaesthetic consultation
4Report any anticoagulant or antiplatelet treatment
5Fast for 6 hours before surgery
6Stopping smoking a few weeks beforehand is advised

Procedure

1

General anaesthesia, or regional anaesthesia depending on the technique and the patient

2

Incision in the groin crease, or three small incisions for laparoscopy

3

Identification of the hernia sac and return of its contents into the abdomen

4

Placement of a soft mesh to reinforce the wall

5

Closure of the incisions, often with absorbable stitches

6

Getting up and walking within a few hours

Duration

45 minutes to 1 hour 30 minutes

Aftercare & Recovery

Discharge takes place the same day or the next day. Walking is immediate, daily activities resume within a few days and office work within one to two weeks. Heavy effort and sport are resumed gradually, generally after three to four weeks, according to the surgeon's advice. A follow-up consultation is scheduled.

Risks and side effects

Groin pain and discomfort for a few days, relieved by painkillers
Bruising or a small fluid collection (seroma), which usually resolves on its own
Persistent groin pain, uncommon, managed in consultation
Infection of the wound or the mesh, rare
Recurrence is possible but uncommon with mesh reinforcement techniques

These risks remain rare. Your surgeon will inform you in detail before the procedure.
