Gastrectomy (partial or total removal of the stomach)

Gastrectomy in Tunis, Tunisia · Centre Urbain Nord

Removal of part or all of the stomach, mainly for a tumour, with reconstruction of the digestive tract.

What is it?

Gastrectomy is the removal of part of the stomach (partial or subtotal gastrectomy) or all of it (total gastrectomy). It is most often performed for stomach cancer, sometimes for a benign tumour or a gastrointestinal stromal tumour (GIST), and more rarely for a complicated ulcer. It is different from sleeve gastrectomy, which is a weight-loss operation. The operation is performed through open surgery or laparoscopy depending on the situation. For cancer, the surgeon removes the diseased part of the stomach with a safety margin, together with the nearby lymph nodes (lymphadenectomy). The digestive tract is then reconstructed: the remaining stomach, or the oesophagus after total gastrectomy, is joined to a loop of small intestine. For cancer, the decision is taken at a multidisciplinary team meeting bringing together surgeons, oncologists, gastroenterologists and radiologists. Chemotherapy before and/or after surgery is often recommended. Nutritional status is assessed and improved before surgery if needed. At Clinique Pasteur Tunis, gastrectomy is supported by a full assessment (gastroscopy with biopsies, CT scan), nutritional preparation, available intensive care and dietary follow-up after discharge, which is essential for learning to eat again.

When is it indicated?

Stomach cancer

This is the main indication: removing the tumour with nearby lymph nodes is the cornerstone of treatment, often combined with chemotherapy.

Stromal tumour (GIST) or benign tumour

Some tumours of the stomach wall are treated by removing a limited part of the stomach, sometimes laparoscopically.

Complicated ulcer not responding to treatment

Rarely nowadays, an ulcer that bleeds, narrows the stomach or recurs despite medication may justify a partial gastrectomy.

Extensive precancerous lesions

In certain specific cases, lesions with a high risk of becoming cancerous lead to removal of the stomach being offered.

Aftercare & Recovery

The hospital stay usually lasts 7 to 12 days. Eating resumes gradually, with small, frequent meals. Weight loss is common in the first months; dietary follow-up helps with adjustment. After total gastrectomy, vitamin B12 injections are needed for life. Follow-up is provided by the surgeon and the oncologist.

Risks and side effects

Leakage from a join (fistula), a serious complication closely monitored
Infection, bleeding or chest complications after surgery
Early fullness and discomfort after meals (dumping), which improve with an adapted diet
Weight loss and deficiencies (iron, vitamin B12), prevented by follow-up
General complications of major surgery, managed in intensive care if needed

These risks remain rare. Your surgeon will inform you in detail before the procedure.
