Emergency laparotomy (obstruction, perforation, peritonitis)

Emergency laparotomy in Tunis, Tunisia · Centre Urbain Nord

Emergency operation to treat a bowel obstruction, a digestive perforation or peritonitis, through an incision or by laparoscopy.

What is it?

Emergency laparotomy is an opening of the abdomen performed quickly to treat a serious cause of abdominal pain: bowel obstruction, perforation of an organ (perforated ulcer, perforated diverticulitis), peritonitis (infection of the membrane lining the abdomen), internal bleeding or trauma. When possible and safe, the same operation is performed laparoscopically. The surgeon explores the whole abdomen, finds the cause and treats it: stitching a perforated ulcer, removing a damaged segment of bowel, relieving an obstruction, controlling bleeding. The abdominal cavity is then washed out thoroughly. Depending on the state of the bowel and the infection, the digestive tract is rejoined at the same time or a temporary stoma is created, to be closed later. This surgery is required when there are signs of severity: intense pain, a rigid abdomen, fever, vomiting, inability to pass wind, feeling faint. Diagnosis is based on examination, blood tests and a CT scan. The speed of treatment directly affects recovery. At Clinique Pasteur Tunis, the emergency department is open around the clock, CT scanning is available on site and the operating theatres work day and night. The general intensive care unit takes over when the patient's condition requires it.

When is it indicated?

Peritonitis

When infection has spread inside the abdomen, surgery is needed to treat its cause and wash out the abdominal cavity.

Perforation of a digestive organ

A perforated ulcer or diverticulum lets digestive contents leak into the abdomen; the hole must be closed or the segment removed.

Bowel obstruction with signs of severity

A blocked bowel that is suffering must be freed quickly, with removal of the damaged part if necessary.

Internal bleeding or abdominal trauma

After a violent blow or with bleeding inside the abdomen, surgery finds and controls the source of bleeding.

Intestinal ischaemia

When the bowel suddenly loses its blood supply, the parts that cannot recover must be removed.

4Immediate assessment by the anaesthetist
5Information for the patient and family about the planned operation

Treatment of the cause: suture, resection, release or control of bleeding

Aftercare & Recovery

Recovery depends on the cause and general health. Monitoring in intensive care is sometimes needed for the first days. The hospital stay usually lasts one to two weeks. Convalescence at home takes several weeks; if a stoma was created, its closure is planned later.

Risks and side effects

Wound infection or abdominal abscess, sometimes drained
Slow return of bowel function
Leakage from a bowel join
Incisional hernia in the scar later on
General complications (chest, infection), managed in intensive care

These risks remain rare. Your surgeon will inform you in detail before the procedure.
