Haemorrhoidectomy (Milligan-Morgan technique)

Haemorrhoidectomy in Tunis, Tunisia · Centre Urbain Nord

Surgical removal of large or complicated haemorrhoids, the reference technique with long-lasting results.

What is it?

Haemorrhoids are veins that are normally present in the anal canal. They become a problem when they enlarge, bleed, come out of the anus (prolapse) or become thrombosed. Haemorrhoidectomy means surgically removing the diseased haemorrhoidal cushions. The Milligan-Morgan technique is the most widely used and best studied. Under anaesthesia, the surgeon usually removes the three main haemorrhoidal cushions, leaving bridges of healthy skin and lining between them, which are essential for good healing. The wounds are usually left open and heal gradually over a few weeks. This operation is offered when medical treatment (a high-fibre diet, gentle laxatives, venotonic medication) and outpatient procedures (rubber band ligation, photocoagulation) are no longer enough, or straight away for very large haemorrhoids. It is the technique with the lowest risk of recurrence. At Clinique Pasteur Tunis, haemorrhoidectomy is performed in the operating theatre, with particular attention to pain relief after surgery and clear advice on wound care and bowel habits at home.

When is it indicated?

Grade 3 or 4 haemorrhoids

When haemorrhoids come out with every bowel movement and have to be pushed back with a finger, or stay out permanently, surgery is the most effective solution.

Repeated bleeding despite medical treatment

Frequent bleeding can cause tiredness or anaemia. When simple treatments fail, removing the diseased cushions usually stops it for the long term.

Failure of rubber band ligation or photocoagulation

These outpatient procedures suit small haemorrhoids. If symptoms come back, surgery takes over.

Recurrent thrombosed haemorrhoids

Repeated painful episodes, with clots forming in the haemorrhoids, often justify a definitive treatment.

Haemorrhoids combined with a fissure or troublesome skin tags

The surgeon can treat an anal fissure at the same time or remove skin folds (skin tags) that make hygiene difficult.

Aftercare & Recovery

The hospital stay is usually 1 to 2 days. Pain in the first few days is treated with suitable painkillers and sitz baths. It is essential to keep stools soft with the prescribed laxatives. Complete healing takes 4 to 6 weeks; a return to office work is possible after 1 to 3 weeks. Follow-up appointments check the healing process.

Risks and side effects

Pain in the first few days, especially with bowel movements, relieved by the prescribed treatment
Moderate bleeding, mainly around day 10 when the scabs fall off
Temporary difficulty passing urine after surgery
Narrowing of the anus or continence problems, which are rare
Delayed healing, monitored at follow-up

These risks remain rare. Your surgeon will inform you in detail before the procedure.
