Complex anal fistula surgery

Complex anal fistula in Tunis, Tunisia · Centre Urbain Nord

Staged treatment of a deep anal fistula, with seton placement and techniques that preserve the anal muscle.

What is it?

An anal fistula is called complex when its tract crosses a large part of the anal muscle (sphincter), has branches, keeps recurring, or occurs in a particular setting (Crohn's disease, previous anal surgery, a fistula at the front of the anus in women). In these situations, simply laying the tract open could affect continence. Treatment is therefore carried out in stages. The first stage often involves draining the infection and placing a seton: a soft thread passed through the tract and tied outside, which provides continuous drainage and calms the inflammation for several weeks. In a second stage, the surgeon chooses the most suitable technique to close the tract while preserving the sphincter: an advancement flap that covers the internal opening, ligation of the tract between the muscles (LIFT technique), laser closure of the tract, or keeping the seton in place for longer. The choice depends on the anatomy of the fistula, as shown on pelvic MRI. At Clinique Pasteur Tunis, these fistulas are treated step by step, with appropriate imaging and, when needed, in coordination with the gastroenterologist. Patients are informed at each stage of the aims and timing of treatment.
Complex anal fistula surgery

When is it indicated?

Fistula crossing a large part of the sphincter

When the tract passes through a thick part of the anal muscle, techniques that protect this muscle are needed to preserve continence.

Branched fistula or fistula with several openings

A tract that divides or opens in several places requires a precise assessment and staged treatment.

Fistula recurring after previous surgery

A fistula that comes back despite a first treatment calls for a tailored strategy, often guided by MRI.

Fistula associated with Crohn's disease

In this setting, surgery mainly aims to drain the infection and protect the sphincter, alongside treatment of the disease by the gastroenterologist.

Anterior fistula in women

At the front of the anus the sphincter is thinner in women, so a muscle-sparing technique is preferred.

Preparation

1Surgical consultation with a proctological examination
2Pelvic MRI to map the tract precisely
3Opinion from the gastroenterologist in case of inflammatory bowel disease
4Pre-operative blood tests and anaesthetic consultation
5Rectal preparation and 6 hours of fasting as instructed

Procedure

1

General or spinal anaesthesia

2

Precise exploration of the tract and any branches

3

Drainage of infected collections

4

Placement of a draining seton during the first stage

5

In a second stage, closure of the tract with the chosen technique (flap, LIFT, laser…)

6

Monitoring in the recovery room, then on the ward

Duration

30 minutes to 1.5 hours depending on the stage

Aftercare & Recovery

Each stage usually requires a short hospital stay of 1 to 2 days. The seton is well tolerated and stays in place for several weeks. Complete healing can take several months and more than one procedure. Regular appointments track progress and plan the next stage.

Risks and side effects

Discharge and local discomfort while the seton is in place
Persistence or recurrence of the fistula, which may require a further procedure
Continence problems, which sphincter-sparing techniques aim to prevent
Delayed healing
Bleeding or local infection, monitored at follow-up

These risks remain rare. Your surgeon will inform you in detail before the procedure.
