Stoma closure (restoring bowel continuity)

Stoma closure in Tunis, Tunisia · Centre Urbain Nord

Operation to close an ileostomy or colostomy and restore the natural passage of stools.

What is it?

A stoma is an opening of the bowel onto the skin of the abdomen, allowing stools to drain into a bag. It is sometimes created temporarily: to protect a fragile join (after rectal surgery, for example), or after an emergency operation (peritonitis, obstruction, perforated diverticulitis). When the situation allows, it is closed to restore the normal digestive tract. Closing a loop ileostomy or loop colostomy is a short operation: the surgeon frees the bowel around the opening, joins the two segments and closes the abdominal wall. After a Hartmann's procedure (end colostomy with the rectum closed), reversal is a bigger operation: the rectum must be found and joined to the colon, often laparoscopically. Before closure, the team checks that the protected join has healed properly (contrast X-ray, endoscopy) and that the person is in good general health, usually after a few weeks to a few months. The surgeon also assesses how well the anal sphincter works. At Clinique Pasteur Tunis, the date of closure is set in consultation after these checks. The team supports the return of natural bowel function, with dietary advice for the first weeks.

When is it indicated?

Protective ileostomy after rectal surgery

Once the rectal join has healed and been checked, the stoma is no longer needed and can be closed.

Colostomy after a Hartmann's procedure

After emergency surgery in which the rectum was closed, the colon can be rejoined to the rectum once the person has recovered.

Temporary stoma after peritonitis or obstruction

When the infection has cleared and general health has recovered, the natural route can be restored.

Wish to improve quality of life

Getting rid of the bag is a real relief in daily life, provided tests show it can be done safely.

Preparation

1Check of the join by contrast X-ray or endoscopy
2Assessment of anal continence
3Blood tests and anaesthetic consultation
4Bowel preparation through the stoma and/or the anus according to instructions
5Fast for 6 hours before surgery

Procedure

1

General anaesthesia

2

Incision around the stoma, or laparoscopy for Hartmann reversal

3

Release of the bowel and adhesions

4

Joining of the two bowel segments

5

Closure of the abdominal wall at the former stoma site

6

Skin closure, sometimes partial to limit the risk of infection

Duration

1 hour (ileostomy) to 3 hours (after Hartmann's procedure)

Aftercare & Recovery

The hospital stay usually lasts 3 to 7 days, until bowel function returns. In the first weeks, stools may be frequent and irregular; they settle gradually with an adapted diet. Lifting is avoided for about six weeks to protect the abdominal wall.

Risks and side effects

Slow return of bowel function for a few days
Infection of the former stoma wound
Frequent or urgent stools at first
Leakage from the join, uncommon, closely monitored
Hernia at the former stoma site later on

These risks remain rare. Your surgeon will inform you in detail before the procedure.
