Total colectomy and proctocolectomy

Total colectomy in Tunis, Tunisia · Centre Urbain Nord

Removal of the entire colon, sometimes with the rectum, for ulcerative colitis, familial polyposis or multiple cancers.

What is it?

Total colectomy is the removal of the whole colon, with the small intestine then joined to the rectum (ileorectal anastomosis). When the rectum must also be removed, the operation is called a proctocolectomy: a pouch is made from the small intestine and joined to the anus (ileal pouch-anal anastomosis), so that stools can still be passed naturally. These operations are performed laparoscopically or through open surgery, depending on the situation and the urgency. During a proctocolectomy, a temporary stoma (protective ileostomy) is often created for a few weeks to protect the pouch while it heals; it is closed later during a short second operation. They are offered for ulcerative colitis that resists medical treatment or is complicated, familial adenomatous polyposis (an inherited disease in which hundreds of polyps form in the colon), some multiple colon cancers, or as an emergency for severe colitis. The decision is made after discussion with the gastroenterologist and, in the case of cancer, at a multidisciplinary meeting. At Clinique Pasteur Tunis, preparation includes detailed information on life after surgery (bowel habits, diet, temporary stoma), and support from the nursing team for stoma care when one is needed.

When is it indicated?

Resistant or complicated ulcerative colitis

When medication no longer controls the disease, or if precancerous changes appear, removing the colon and rectum puts an end to the disease of the colon and rectum.

Familial adenomatous polyposis

In this inherited disease the risk of cancer is very high; preventive removal of the colon protects the person.

Multiple cancers or polyps of the colon

When several lesions are spread along the colon, it may be safer to remove the whole colon in a single operation.

Severe colitis as an emergency

A very severe flare that does not respond to intensive treatment may require emergency colectomy with a temporary stoma.

1Colonoscopy, CT scan and MRI depending on the case

Aftercare & Recovery

The hospital stay usually lasts 7 to 10 days. Stools are more frequent than before (often 4 to 8 a day) and decrease gradually over the months. An adapted diet helps to thicken stools. When a stoma has been created, its closure is planned after a few weeks to a few months.

Risks and side effects

Frequent and sometimes urgent stools, which improve with time
Leakage from a join or a collection, closely monitored
Bowel obstruction due to adhesions, later on
Inflammation of the pouch (pouchitis), treated with antibiotics
Dehydration with a high-output stoma, prevented by the team's advice

These risks remain rare. Your surgeon will inform you in detail before the procedure.
