Upper GI series and small bowel follow-through in Tunis: X-rays taken after drinking a contrast liquid, for the oesophagus, stomach and bowel.

What is it?

An upper GI series (barium swallow and meal) is an X-ray examination of the oesophagus, stomach and first part of the small intestine (duodenum), carried out while you drink a liquid that shows up on X-rays. The radiologist follows its passage live on a screen. Depending on the question, the liquid is barium (a thick white liquid) or a water-soluble iodinated contrast agent, especially to check a suture after stomach surgery (for example weight-loss surgery) or when a leak is suspected. A pharyngo-oesophageal study looks more closely at swallowing; the upper GI series may be extended to the small intestine, or a small bowel follow-through may be done on its own. Today, gastroscopy and CT scans answer many questions, but contrast studies remain useful for assessing the movement and shape of the organs.

When is it indicated?

Swallowing problems

The pharyngo-oesophageal study shows how food passes from the mouth into the oesophagus.

Reflux and hiatus hernia

It may show stomach contents flowing back up, or part of the stomach passing above the diaphragm.

Check after stomach surgery

With a water-soluble agent, it checks that sutures are watertight and that liquid passes properly, for example after weight-loss surgery.

Study of the small intestine

The small bowel follow-through looks for a narrowing or abnormality of the bowel, as indicated by the doctor.

Preparation

1Tell us before the examination if you are pregnant or think you might be
2Fast (no food, drink or smoking) as instructed when you book, usually from the evening before
3Tell us about any allergy to iodinated contrast or kidney disease if a water-soluble agent is planned
4Tell us about any stomach or oesophageal surgery and bring the operative report
5Remove jewellery, watch, belt, piercings and any metal object located in the area to be X-rayed
6Bring your referral and any previous X-rays or reports of the same area

Procedure

1

You remove clothing and metal objects from the area being examined, in a changing cubicle

2

You stand or lie in front of the machine and are given the liquid to drink in small sips

3

The radiologist follows the contrast agent live on a screen (fluoroscopy) and takes images at the right moments

4

You lie on an X-ray table that can tilt; you may be asked to change position

5

For the small bowel follow-through, images are repeated at regular intervals until the liquid reaches the end of the small intestine

6

After the examination, drink plenty of water; after barium, stools may be whitish for a few days

Duration

About 20 to 30 minutes for an upper GI series; several hours for a small bowel follow-through, with spaced images

Results

The images are analysed by a radiologist, who writes a report. It is given to you with the images and sent to your doctor, who will explain the results and the next steps. You will be told on the day of the examination when the results will be ready.

Precautions and contraindications

Higher X-ray dose than a plain X-ray, because images are taken over several minutes, but kept as low as possible
Not performed during pregnancy, except in specific medical circumstances
Barium may cause constipation: drink plenty of fluids and see a doctor if bowel movements do not return
If a perforation or leak is suspected, a water-soluble iodinated agent is used instead of barium
Rarely, the swallowed liquid may go into the airways (aspiration), especially with swallowing problems: the team monitors the examination

Your doctor and the radiology team will inform you in detail before the examination. Feel free to ask them your questions.

This information is general and does not replace the advice of your doctor, who chooses the examination suited to your situation.
