Lower jaw osteotomy (sagittal split osteotomy)

Mandibular osteotomy in Tunis, Tunisia · Centre Urbain Nord

Moving the lower jaw forward or back to correct a jaw mismatch, combined with orthodontic treatment.

What is it?

A lower jaw (the mandible) that is set too far back gives a receding chin, with the lower teeth sitting well behind the upper ones; a mandible that is too far forward gives a prominent chin, with the lower teeth in front of the upper ones. A mandibular osteotomy moves the lower jaw into the correct position. The most widely used technique is the bilateral sagittal split osteotomy. It is performed under general anaesthesia, through incisions inside the mouth, behind the last molars: no scar is visible. The bone is cut on each side, the tooth-bearing part is moved forward or back according to the plan, then secured with small titanium plates or screws. It is usually part of a treatment combining an orthodontist and a maxillofacial surgeon. It may be performed alone or together with an upper jaw osteotomy, and is sometimes completed by a genioplasty (repositioning of the chin). At Clinique Pasteur Tunis, this surgery is performed by maxillofacial surgeons in the operating theatre, in coordination with the patient's orthodontist, with suitable monitoring after the operation.

When is it indicated?

Lower jaw set too far back (retrognathia)

When the chin recedes and the lower teeth sit well behind the upper ones, moving the mandible forward restores the bite and balances the profile.

Lower jaw too far forward (prognathism)

When the lower teeth close in front of the upper ones, moving the mandible back makes normal biting and chewing possible.

Asymmetry of the lower jaw

A chin that deviates to one side can be recentred for a more symmetrical bite and face.

Chewing problems caused by the mismatch

When the teeth do not fit together, cutting and chewing food becomes difficult; surgery improves this function.

Sleep apnoea related to a receding jaw

In some cases, moving the jaw forward widens the airway in the throat, which can improve breathing at night.

Preparation

1Preparatory orthodontic treatment, often lasting several months

Aftercare & Recovery

The hospital stay usually lasts 2 to 3 days. Swelling of the lower face is significant in the first few days, then decreases gradually over several weeks. A liquid then blended diet is followed for several weeks. Mouth opening returns gradually, sometimes with the help of exercises. Return to office work is possible after 2 to 3 weeks, and the orthodontist then finalises the bite.

Risks and side effects

Temporary swelling and bruising
Reduced sensation of the lower lip and chin, usually temporary, sometimes lasting
Temporarily limited mouth opening
Infection or discomfort related to the plates, sometimes requiring their removal
Slight relapse of the mismatch, monitored and corrected with orthodontics

These risks remain rare. Your surgeon will inform you in detail before the procedure.
