Surgery for facial fractures (jaw, cheekbone, nose)

Facial fractures in Tunis, Tunisia · Centre Urbain Nord

Realignment and fixation of facial fractures after an injury, to restore the bite, function and balance of the face.

What is it?

A blow to the face, from a fall, a road accident, an assault or a sports injury, can fracture the bones of the face: the mandible (lower jaw), the zygomatic bone (cheekbone), the nasal bones, or the floor of the eye socket. These fractures can interfere with chewing, breathing or vision, and change the appearance of the face. Treatment consists of putting the bone fragments back in place (reduction) and holding them while they heal. For a nasal fracture, reduction is often done without an incision, by repositioning the bones from inside, then a splint is applied. For the jaw or cheekbone, the surgeon usually works through incisions inside the mouth or in discreet skin creases, and fixes the bone with small titanium plates and screws. Temporary wiring of the jaws is sometimes needed. Surgery is offered when the fracture is displaced, when it disturbs the bite, nasal breathing or the position of the eye, or when it leaves a visible deformity. Some minimally displaced fractures are simply monitored. At Clinique Pasteur Tunis, patients can be received by the emergency department, have a facial CT scan on site, and then be operated on in the operating theatre by maxillofacial or ENT surgeons depending on the fracture.

When is it indicated?

Fracture of the lower jaw (mandible)

When the teeth no longer fit together as before after a blow, the jaw is often broken; fixing it allows normal eating and speaking.

Cheekbone fracture (zygomatic bone)

A flattened cheekbone changes the appearance of the face and can limit mouth opening or affect vision; putting it back in place corrects these problems.

Displaced fracture of the nasal bones

A nose that is crooked or blocked after a knock can be straightened, ideally in the days that follow, once the swelling has gone down.

Fracture of the floor of the eye socket

Double vision or a sunken eye after an injury may require repair of the bony floor of the eye socket.

Combined fractures of several facial bones

After a severe injury, several bones may be affected; they are repaired in a precise order to rebuild the face.

Preparation

1Full clinical examination of the face, teeth and eyes
2CT scan of the face to define the fractures
3Anaesthesia consultation and blood tests

5Fasting for 6 hours before the operation

Procedure

1

General anaesthesia, sometimes local anaesthesia for a simple nasal fracture

2

Access to the fracture through the mouth, the nostrils or a discreet incision

3

Repositioning of the bone fragments

4

Fixation with small titanium plates and screws if needed

5

Check of the dental bite and facial symmetry

6

Possible nasal splint or temporary wiring of the jaws

7

Monitored recovery, then back to the ward

Duration

30 minutes to 3 hours, depending on the type and number of fractures

Aftercare & Recovery

The hospital stay ranges from a few hours for a nasal fracture to a few days for a jaw fracture. Swelling and bruising decrease within 1 to 2 weeks. A soft or blended diet is needed for several weeks after a mandibular fracture. Contact sports should be avoided for at least 6 weeks. Follow-up visits, with imaging if needed, check that the bone has healed.

Risks and side effects

Temporary swelling and bruising
Reduced sensation of the lip, cheek or nose, usually temporary
Residual bite problem, sometimes corrected with orthodontics
Infection or discomfort related to the plates, sometimes requiring their removal
Residual asymmetry that may need a touch-up procedure

These risks remain rare. Your surgeon will inform you in detail before the procedure.
