Pilonidal cyst (pilonidal sinus)

Pilonidal cyst in Tunis, Tunisia · Centre Urbain Nord

Surgical treatment of a pilonidal cyst in the natal cleft, during an abscess or as a planned operation, with suitable wound care.

What is it?

A pilonidal cyst (or pilonidal sinus) is a cavity under the skin of the cleft between the buttocks, at the top of the groove, often containing hairs. It mainly affects young adults, more often men, and is encouraged by abundant body hair and prolonged sitting. It shows as small openings in the cleft, a discharge, or a painful, swollen abscess. In the case of an abscess, a quick procedure to open and drain it relieves the pain; definitive treatment is carried out later, once things have calmed down. It consists of removing the cyst and its tracts completely. The wound may be left open to heal gradually with nursing care, or closed, possibly shifting the scar away from the cleft with a small skin flap to reduce the risk of recurrence. Less extensive techniques, which remove only the openings and clean the cavity, are also possible for limited forms. Surgery is offered for cysts that become infected, discharge or hurt. A cyst that has never caused symptoms can simply be monitored. The technique is chosen in consultation, according to the extent, any recurrences and the person's constraints. At Clinique Pasteur Tunis, the operation is usually done as day surgery or with one night in hospital. Precise instructions are given on dressings, hygiene and hair removal in the area, which play an important role in preventing recurrence.

When is it indicated?

Abscess in the natal cleft

A hot, red, very painful lump at the top of the buttock cleft must be opened and drained quickly to relieve pain.

Persistent discharge or openings

Small holes in the cleft that leak fluid indicate an active cyst that is best removed.

Repeated abscesses

After one or more abscesses, complete removal of the cyst as a planned operation limits the risk of new infection.

Recurrence after a previous operation

A cyst that comes back can be operated on again, often with a technique that moves the scar away from the cleft.

Aftercare & Recovery

Discharge is the same day or the next day. If the wound is left open, daily dressings by a nurse are needed for several weeks until it has fully healed. If it is closed, healing is faster but prolonged sitting is avoided for a few weeks. Regular hair removal in the area is advised afterwards.

Risks and side effects

Slow healing when the wound is left open
Wound breakdown or infection when the wound is closed
Pain when sitting in the first weeks
Possible recurrence, reduced by hygiene and hair removal in the area

These risks remain rare. Your surgeon will inform you in detail before the procedure.
