Transcatheter edge-to-edge tricuspid valve repair (TriClip)

Transcatheter tricuspid repair (TriClip) in Tunis, Tunisia · Centre Urbain Nord

Catheter reduction of a severe tricuspid valve leak, bringing its leaflets together with one or more clips.

What is it?

The tricuspid valve lies in the right side of the heart, between the right atrium and right ventricle. When it leaks severely, blood flows back into the veins: swollen legs, a swollen abdomen, a congested liver, fatigue and breathlessness. Tricuspid regurgitation is often linked to enlargement of the right heart, atrial fibrillation or pacemaker leads. Isolated tricuspid valve surgery is often a major operation in these patients. Transcatheter edge-to-edge repair applies to the tricuspid valve the same principle as for the mitral valve: one or more small clips bring the leaflets together to reduce the leak. The system is brought through the femoral vein at the groin to the right side of the heart, without crossing the wall between the atria. The procedure is performed under general anaesthesia, guided by transoesophageal echocardiography and X-ray imaging. The indication is decided by a multidisciplinary team (the "heart team") for patients with severe tricuspid regurgitation and symptoms despite well-conducted medical treatment, especially diuretics, when surgery is considered high risk and the valve anatomy is suitable. It is a more recent technique whose main aim is to improve symptoms and quality of life. At Clinique Pasteur Tunis, the decision is based on this joint discussion, and the procedure brings together interventional cardiologists, an echocardiographer and anaesthetists, in a latest-generation catheterisation laboratory, with cardiac intensive care on site.

When is it indicated?

Severe symptomatic tricuspid regurgitation

Swollen legs, a swollen abdomen and fatigue reflect an overloaded right heart, which reducing the leak can improve.

Symptoms persisting despite diuretics

When the medicines that remove excess fluid are no longer enough or become poorly tolerated, repair can be considered.

High surgical risk

Open-heart surgery may be too risky for some patients; the catheter approach avoids opening the chest.

Favourable valve anatomy

Transoesophageal echocardiography checks the shape and spacing of the leaflets, as well as the position of any pacemaker leads.

Preparation

1Detailed transthoracic and transoesophageal echocardiography of the tricuspid valve
2Heart failure assessment and sometimes right heart catheterisation to measure pressures
3Blood tests, including kidney and liver function, and anaesthetic consultation
4Heart team review and information for the patient
5Adjustment of anticoagulants as instructed and fasting before the procedure

Procedure

1

General anaesthesia and placement of the transoesophageal echocardiography probe

2

Puncture of the femoral vein at the groin

3

Advancement of the delivery system to the right atrium

4

Positioning and placement of one or more clips on the leaflets, guided by echocardiography

5

Check of the reduction in the leak

6

Removal of the system, closure of the puncture site and recovery from anaesthesia

Duration

About 2 to 3 hours

Results

Patients usually get up the next day, with a usual hospital stay of 2 to 4 days. Symptoms (swelling, breathlessness) improve gradually, and diuretic doses can sometimes be reduced. Antithrombotic treatment, follow-up echocardiograms and regular cardiology follow-up are arranged. Some residual leak may persist.

Risks and side effects

Bruising at the groin puncture site, generally minor
Residual leak despite the clip or clips
Partial detachment of a clip, uncommon, monitored by echocardiography
Rare rhythm disturbances or cardiac complications, managed by the team

These risks remain rare. Your physician will inform you in detail before the examination.
