Transcatheter edge-to-edge mitral valve repair (MitraClip)

Transcatheter mitral repair (MitraClip) in Tunis, Tunisia · Centre Urbain Nord

Catheter treatment of a leaking mitral valve, bringing its two leaflets together with a small clip, without opening the chest.

What is it?

The mitral valve separates the left atrium from the left ventricle. When it no longer closes properly, some blood flows backwards with each heartbeat: this is mitral regurgitation. When severe, it causes breathlessness and fatigue and can lead to heart failure. Surgical repair remains the reference treatment when possible, but some patients are too frail for open-heart surgery. Transcatheter edge-to-edge repair attaches the two valve leaflets together at the site of the leak, using one or more small fabric-covered metal clips. The clip is brought through a vein at the groin, then crosses from the right atrium to the left atrium through the wall that separates them (transseptal puncture). The procedure is performed under general anaesthesia and is continuously guided by transoesophageal echocardiography and X-ray imaging. The indication is decided by a multidisciplinary team (the "heart team") of cardiologists, cardiac imaging specialists, cardiac surgeons and anaesthetists. It concerns severe, symptomatic mitral regurgitation when surgery is considered high risk, or certain leaks caused by an enlarged heart that persist despite optimal medical treatment. At Clinique Pasteur Tunis, the decision is based on this joint discussion, and the procedure is performed by a team of interventional cardiologists, an echocardiographer and anaesthetists, in a latest-generation catheterisation laboratory, with cardiac intensive care on site.

When is it indicated?

Severe mitral regurgitation with breathlessness

When the valve lets a lot of blood flow back and the patient becomes short of breath or tired quickly, reducing the leak relieves the heart.

High surgical risk

Advanced age, other illnesses or frailty can make open-heart surgery too risky; the catheter approach is then an alternative.

Leak due to an enlarged heart despite medical treatment

In some forms of heart failure, the leak persists despite well-adjusted medication; the clip can reduce hospital admissions and improve symptoms in selected patients.

Suitable valve anatomy

Transoesophageal echocardiography checks that the shape of the leaflets allows them to be grasped and brought together effectively.

Heart team decision

The indication is discussed and agreed collectively, with the patient, after comparison with surgery and medical treatment.

Preparation

1Transthoracic and transoesophageal echocardiography to analyse the leak and the valve
2Full cardiac assessment, often including coronary angiography
3Blood tests and anaesthetic consultation
4Heart team review and detailed information for the patient
5Adjustment of anticoagulants and diuretics as instructed, fasting before the procedure

Procedure

1

General anaesthesia and placement of the transoesophageal echocardiography probe

2

Puncture of the femoral vein at the groin

3

Crossing from the right atrium to the left atrium through the interatrial septum

4

Positioning of the clip above the leak, guided by three-dimensional echocardiography

5

Grasping and joining of the two leaflets, check of the leak, placement of a second clip if needed

6

Removal of the system, closure of the puncture site and recovery from anaesthesia

Duration

About 2 to 3 hours

Results

Patients usually get up the next day. The hospital stay is usually 2 to 4 days. Breathlessness improves gradually over a few weeks. Antithrombotic treatment is prescribed, together with follow-up echocardiograms and regular heart failure follow-up. The result depends on each patient's valve and heart; some residual leak may persist.

Risks and side effects

Bruising or bleeding at the venous puncture site, usually minor
Residual leak or mild narrowing of the valve after clip placement
Partial detachment of the clip, uncommon, monitored by echocardiography
Small persistent opening between the atria, generally without consequence
Rare serious complications (fluid around the heart, stroke), managed by the team

These risks remain rare. Your physician will inform you in detail before the examination.
