Transcatheter left atrial appendage closure

Left atrial appendage closure in Tunis, Tunisia · Centre Urbain Nord

Catheter placement of a device that closes the left atrial appendage, to prevent stroke from atrial fibrillation when anticoagulants are not possible.

What is it?

The left atrial appendage is a small dead-end pouch attached to the left atrium. In atrial fibrillation, the atrium no longer contracts effectively and blood can stagnate in this pouch, where the vast majority of clots responsible for strokes form. The usual treatment to prevent these strokes is long-term anticoagulant medication. In some patients, this treatment is not possible or is dangerous, particularly after serious bleeding. Left atrial appendage closure then seals the entrance of the pouch with an umbrella- or plug-shaped wire-mesh device. It is brought through a vein at the groin and then into the left atrium across the interatrial septum (transseptal puncture). The procedure is guided by X-ray imaging and transoesophageal or intracardiac echocardiography, under general anaesthesia or sedation. The decision is made after careful assessment of the stroke risk and the bleeding risk, by cardiologists together, sometimes with the neurologist or gastroenterologist depending on the source of bleeding. Lighter antithrombotic treatment is still needed for a few weeks to a few months after implantation, until the device is covered by heart tissue. At Clinique Pasteur Tunis, this procedure is performed in a latest-generation catheterisation laboratory by an interventional cardiology team, supported by echocardiography and with a cardiac surgery team on site.

When is it indicated?

Atrial fibrillation with a high stroke risk

When age and other illnesses increase the risk of clots, protection is needed; the device provides it when anticoagulants are not possible.

Previous serious bleeding on anticoagulants

After a serious digestive or brain haemorrhage, restarting long-term anticoagulation may be too dangerous.

Lasting contraindication to anticoagulants

Some illnesses or situations make long-term anticoagulation impossible; closing the appendage reduces the need for this treatment.

Atrial fibrillation not related to significant valve disease

The technique is intended for atrial fibrillation without severe mitral stenosis or a mechanical valve, situations that require an anticoagulant.

Preparation

1Transoesophageal echocardiography or cardiac CT to measure the appendage and rule out a clot
2Assessment of stroke risk and bleeding risk by the cardiologist
3Blood tests and anaesthetic consultation
4Adjustment of anticoagulant or antiplatelet treatment as instructed
5Fasting before the procedure

Procedure

1

General anaesthesia or sedation, set-up of echocardiographic guidance

2

Puncture of the femoral vein at the groin

3

Crossing from the right atrium to the left atrium through the septum

4

Measurement of the appendage and choice of device size

5

Deployment of the device at the entrance of the appendage and check of its stability and seal

6

Release of the device, removal of the equipment and compression of the puncture site

Duration

About 1 hour

Results

Discharge usually takes place the next day after a check echocardiogram. Normal activities resume within a few days. Antithrombotic treatment is prescribed for a few weeks to a few months, then reduced according to the transoesophageal echocardiogram or CT check, usually performed between 6 weeks and 3 months. Regular cardiology follow-up continues.

Risks and side effects

Bruising at the puncture site, usually harmless
Fluid around the heart, uncommon, monitored and treated if needed
Clot forming on the device, uncommon, looked for at check-ups
Device displacement, very rare, which may require retrieval
Small residual leak at the entrance of the appendage, often without consequence

These risks remain rare. Your physician will inform you in detail before the examination.
