Pacemaker or defibrillator generator replacement

Generator replacement in Tunis, Tunisia · Centre Urbain Nord

Planned replacement of a pacemaker or defibrillator generator whose battery is nearing its end, usually keeping the existing leads.

What is it?

Pacemakers and implantable defibrillators run on a battery built into the generator, which cannot be recharged. Their lifespan varies with the model and use, often between 5 and more than 10 years. At regular check-ups, the cardiologist monitors battery wear: when it approaches its end, the device signals it and a replacement is scheduled, not as an emergency but within the indicated time frame. Generator replacement is a short procedure performed under local anaesthesia. The doctor reopens the scar from the original implantation, removes the old generator from its pocket under the skin, disconnects the leads and tests that they work properly. If they are in good condition, they are reconnected to the new generator, which is placed back in the pocket before closure. If a lead is faulty, a new lead can be added at the same time. For a patient who depends on their pacemaker, back-up pacing is arranged during the short disconnection period. For a defibrillator, shock therapies are switched off during the procedure and the heart rhythm is monitored continuously, with an external defibrillator at hand. It is also an opportunity to check that the type of device is still suited to the current condition of the heart. At Clinique Pasteur Tunis, generator replacement is performed by the electrophysiology team under strict aseptic conditions, and follow-up of the new device continues in clinic.

When is it indicated?

Generator battery nearing its end

The device check shows that the battery is reaching its limit; replacement is then scheduled before it runs out.

Generator fault or advisory

Rarely, a generator develops a fault or is subject to a manufacturer advisory; it is then replaced as a precaution.

Need for a different device

If the heart has changed, the replacement can be used to upgrade, for example, to a defibrillator or a resynchronisation device, adding a lead if necessary.

Faulty lead found at check-up

If a lead is no longer working properly, it can be replaced or supplemented by a new one during the same procedure.

Preparation

1Device check confirming battery wear and lead condition
2Blood tests including clotting
3Adjustment of anticoagulants and antiplatelets as instructed by the doctor
4Antiseptic shower the evening before and the morning of the procedure
5Fasting for a few hours before the procedure

Procedure

1

Positioning in a dedicated room, preventive antibiotic and local anaesthesia

2

Reopening of the old scar and removal of the generator from its pocket

3

Disconnection and electrical testing of the leads, with back-up pacing if necessary

4

Connection of the leads to the new generator, or addition of a lead if needed

5

Placement of the generator in the pocket and careful closure

6

Programming and check of the new device, reactivation of defibrillator therapies

Duration

30 minutes to 1 hour

Results

Patients often go home the same day or the next day. The wound must be kept clean and dry; local care and stitch removal, if needed, are explained. Everyday activities resume within a few days, avoiding strain on the arm on the operated side during the first week. Regular device checks continue as before.

Risks and side effects

Bruising of the pocket, more frequent on anticoagulants, usually resolving on its own
Pocket infection, uncommon but to be reported without delay (redness, discharge, fever)
Damage to a lead during the procedure, rare, repaired or replaced at the same time
Temporary local discomfort or pain

These risks remain rare. Your physician will inform you in detail before the examination.
