Implantable loop recorder (implantable cardiac monitor)

Implantable loop recorder in Tunis, Tunisia · Centre Urbain Nord

A small recorder placed under the skin of the chest that monitors the heart rhythm for several years to explain rare fainting or palpitations.

What is it?

The implantable loop recorder, or implantable cardiac monitor, is a miniature electrocardiogram recorder about the size of a small USB stick. Placed just under the skin in front of the heart, it monitors the heart rhythm continuously for about 2 to 3 years, depending on the model. It automatically records rhythm abnormalities and can also be triggered by the patient, using a small activator, at the time of a symptom. Insertion is a quick and simple procedure: under local anaesthesia, the doctor makes a small incision of about one centimetre on the left side of the chest and slides the device under the skin. The small wound is closed with a stitch or skin glue. No lead is placed in the heart. Data are often transmitted remotely, from home, to the cardiology team. It is offered when rare symptoms, such as fainting or occasional palpitations, could not be explained by the usual tests: electrocardiogram, 24- to 48-hour Holter monitoring, echocardiography. It also helps to look for silent atrial fibrillation after a stroke with no cause found. When a rhythm disorder is recorded, the appropriate treatment can be offered: pacemaker, ablation or anticoagulant treatment. At Clinique Pasteur Tunis, insertion is performed by the electrophysiology team, and recordings are analysed during follow-up in clinic.

When is it indicated?

Unexplained recurrent fainting

When episodes are too infrequent to be captured by a standard Holter, recording over several years makes it possible to link them, or not, to a rhythm disorder.

Rare undocumented palpitations

If palpitations occur only a few times a year, the device records them at the moment they happen.

Search for atrial fibrillation after an unexplained stroke

Intermittent, silent atrial fibrillation may have caused the stroke; detecting it changes the preventive treatment.

Rhythm monitoring in certain situations

The cardiologist may suggest it to follow the rhythm after certain treatments or in certain heart conditions.

Preparation

1Cardiology consultation and prior tests (ECG, Holter, echocardiography)
2No fasting required in most cases
3Report anticoagulant or antiplatelet treatment, which is usually continued
4Shower and clean skin on the day of insertion

Procedure

1

Positioning in the room, antiseptic cleaning of the chest skin

2

Local anaesthesia

3

Small incision of about one centimetre

4

Insertion of the device under the skin and check of signal quality

5

Closure with a stitch or skin glue and dressing

6

Explanations about the activator and home data transmission

Duration

About 15 to 20 minutes

Results

Patients go home the same day. The small wound heals in one to two weeks, and usual activities can be resumed almost immediately. Recordings are analysed regularly and with each symptom. When the device is no longer needed or its battery is exhausted, it can be removed by a procedure as simple as its insertion. The device must be mentioned before any MRI scan.

Risks and side effects

Small bruise or local pain, short-lived
Wound infection, uncommon
Minimal cosmetic discomfort due to the slight bump under the skin
Possible false alerts, checked by the cardiologist

These risks remain rare. Your physician will inform you in detail before the examination.
