Intra-aortic balloon pump

Intra-aortic balloon pump in Tunis, Tunisia · Centre Urbain Nord

Temporary circulatory support: a balloon placed in the aorta inflates and deflates in time with the heartbeat to ease the heart's work.

What is it?

The intra-aortic balloon pump is a temporary cardiac support device. It consists of a long balloon placed in the descending thoracic aorta and connected to a console at the bedside. Synchronised with the electrocardiogram, it inflates when the heart relaxes, which improves blood supply to the coronary arteries, then deflates just before the heart contracts, which reduces the effort the heart must make to pump blood out. The balloon is inserted through the femoral artery at the groin, under local anaesthesia, and positioned under X-ray guidance, most often in the catheterisation laboratory. It stays in place for a few hours to a few days, while the heart recovers or a definitive treatment is carried out. The patient is continuously monitored in an intensive care unit, with anticoagulant treatment and the need to keep the affected leg straight. It is used in severe, selected situations: certain mechanical complications of a heart attack (rupture of the septum or acute mitral leak) while awaiting surgery, certain types of cardiogenic shock, very high-risk angioplasty, or severe heart failure awaiting another treatment. Its use is not routine and is decided case by case by the cardiology and intensive care team. At Clinique Pasteur Tunis, insertion takes place in the catheterisation laboratory and monitoring is provided in the cardiac intensive care unit or the cardiac surgical intensive care unit.
Intra-aortic balloon pump

When is it indicated?

Mechanical complication of a heart attack

A rupture of the heart's septum or an acute mitral leak endangers the circulation; the balloon stabilises the patient while awaiting surgery.

Certain types of cardiogenic shock

When the heart can no longer maintain adequate circulation, the balloon can help in situations selected by the team.

Very high-risk angioplasty

When treating vital arteries in a very weakened heart, the balloon can make the procedure safer.

Bridge to cardiac surgery or another treatment

It temporarily supports the heart while an operation or another lasting solution is prepared.

Preparation

1Insertion most often as an emergency or semi-emergency
2Blood tests including clotting and platelet count
3Check of the pulses and condition of the leg arteries
4Information for the patient or family as soon as possible

Procedure

1

Local anaesthesia at the groin

2

Puncture of the femoral artery and placement of a sheath

3

Advancement of the balloon into the descending thoracic aorta under X-ray guidance

4

Connection to the console and synchronisation with the electrocardiogram

5

Position check by X-ray and continuous monitoring in intensive care

6

Removal of the balloon at the bedside once support is no longer needed, followed by compression

Duration

15 to 30 minutes for insertion

Results

The balloon remains in place for a few hours to a few days, in intensive care. Removal is simple and done at the bedside, followed by compression and a few hours of bed rest. The total hospital stay depends mainly on the condition that required the support.

Risks and side effects

Bleeding or bruising at the puncture site, made more likely by the anticoagulant
Poor circulation in the leg, checked regularly by the team
Drop in platelets or infection, uncommon, looked for during monitoring
Complications affecting the aorta, uncommon

These risks remain rare. Your physician will inform you in detail before the examination.
