Hypospadias repair

Hypospadias in Tunis, Tunisia · Centre Urbain Nord

Reconstruction of the urinary channel in boys whose urinary opening is located on the underside of the penis, ideally between 6 and 18 months.

What is it?

Hypospadias is a common condition in boys, present from birth: the opening through which urine comes out (the meatus) is not at the tip of the glans but lower down, on the underside of the penis. It is often associated with an incomplete, "hooded" foreskin and sometimes with a curvature of the penis. Depending on the position of the meatus, hypospadias is described as distal (near the glans, the most common form), mid-shaft or proximal (near the scrotum). The aim of surgery is to move the meatus to the tip of the glans, straighten the penis if needed and give it as natural an appearance as possible, so that the boy can later urinate standing up and have a normal sex life. The operation is generally performed between 6 and 18 months of age. The surgeon rebuilds the missing part of the channel using nearby tissue; simple forms are corrected in one stage, severe forms sometimes in two. A small catheter is often left for a few days to protect the repair. A child with hypospadias should not be circumcised before the surgeon's advice, because the foreskin may be used for the repair. At Clinique Pasteur Tunis, the child is operated on by a paediatric surgeon, with paediatric anaesthesia and suitable pain relief. The team shows the parents the care to be given at home.

When is it indicated?

Urinary opening located on the underside of the penis

Surgery moves the opening to the tip of the glans so that the child can urinate normally, with a straight stream.

Curvature of the penis

An associated curvature is corrected during the operation, which is important for the future.

Incomplete, hooded foreskin

The foreskin is reconstructed or removed, depending on the technique and the parents' wishes, for a harmonious appearance.

Deviated urine stream or difficulty passing urine

A stream that points downwards or sideways is a daily nuisance; the repair straightens it.

Preparation

1Consultation with the paediatric surgeon to examine the child and explain the technique
2No circumcision before the operation
3Sometimes a local hormone treatment for a few weeks beforehand, on prescription, to make the reconstruction easier
4Paediatric anaesthesia consultation
5Fasting according to instructions adapted to age

Procedure

1

General anaesthesia combined with regional anaesthesia (caudal or penile block) for pain relief

2

Straightening of the penis if it is curved

3

Reconstruction of the urinary channel up to the tip of the glans

4

Reconstruction of the glans and the skin of the penis

5

Placement of a small urinary catheter and a dressing

6

Waking up near the parents

Duration

1 to 3 hours depending on the form

Aftercare & Recovery

The hospital stay ranges from one to a few days. The catheter is removed after about 5 to 10 days, depending on the case. The dressing and simple care are explained to the parents. The result can be judged after a few months, once the swelling has gone down. Follow-up continues as the child grows, in particular to check the quality of the stream.

Risks and side effects

Swelling and minor bleeding of the penis for a few days
Fistula: a small abnormal opening along the channel, which may require a touch-up procedure
Narrowing of the meatus or of the reconstructed channel
Infection, rare
Need for a second operation, more common in severe forms

These risks remain rare. Your surgeon will inform you in detail before the procedure.
