Surgery for undescended testicle (orchidopexy)

Undescended testicle in Tunis, Tunisia · Centre Urbain Nord

Surgery to bring down and secure in the scrotum a testicle that has stayed in the groin or abdomen, ideally before 18 months of age.

What is it?

During pregnancy, the testicles form inside the abdomen and then descend into the scrotum. In some boys, one testicle (sometimes both) stops on the way, most often in the groin: this is called an undescended testicle or cryptorchidism. When it has taken an abnormal path, it is called an ectopic testicle. In newborns, the testicle can still descend on its own during the first few months. If it is not in the scrotum by about 6 months of age, surgery is recommended, ideally before 18 months. A testicle left outside the scrotum develops less well; bringing it down early protects its function and makes it easier to examine later in life. The operation, called orchidopexy, is performed through a small incision in the groin crease and another in the scrotum: the surgeon frees the testicle and its cord, closes the small channel that has remained open, and then secures the testicle in the scrotum. When the testicle cannot be felt, laparoscopy (a small camera through the navel) is used to locate it and, depending on the case, bring it down in one or two stages. At Clinique Pasteur Tunis, the child is cared for by a paediatric surgeon with anaesthesia adapted to his age, as a day-case procedure in most cases. Parents receive simple instructions for home care.

When is it indicated?

Testicle not in the scrotum after 6 months of age

After this age, the testicle will no longer descend on its own. The operation brings it to its proper position.

Testicle that can be felt in the groin

This is the most common situation: the testicle is brought down and secured through two small incisions.

Testicle that cannot be felt

When the testicle cannot be felt, a small camera helps find it and choose the best treatment.

Retractile testicle that stays up most of the time

A testicle that sits in the groin most of the time may need to be secured in the scrotum.

Associated inguinal hernia

The small channel that has remained open is closed during the same operation.

Preparation

1Consultation with the paediatric surgeon to examine the child and explain the operation to the parents
2Ultrasound if needed (it does not replace the clinical examination)
3Paediatric anaesthesia consultation
4Fasting according to instructions adapted to age (milk, sugary water)
5Bath the evening before or the morning of the operation

Procedure

1

The child is welcomed with a parent who stays until he falls asleep

2

General anaesthesia combined with local anaesthesia for comfort on waking

3

Small incision in the groin crease to free the testicle and its cord

4

Closure of the channel that has remained open

5

Securing the testicle in the scrotum through a second small incision

6

Laparoscopy if the testicle cannot be felt

7

Waking up near the parents as soon as possible

Duration

45 minutes to 1 hour 30 minutes

Aftercare & Recovery

In most cases the child goes home the same day. The stitches are absorbable. Some swelling of the scrotum is usual for a few days. Cycling, straddle toys and sports should be avoided for about 2 to 3 weeks. The surgeon sees the child again to check the position of the testicle, and follow-up is recommended as he grows.

Risks and side effects

Temporary swelling or small bruise of the scrotum
Wound infection, rare
Testicle moving back up and requiring a second operation, uncommon
Shrinkage of the testicle, rare, mainly for testicles located high up

These risks remain rare. Your surgeon will inform you in detail before the procedure.
