Molecular biology and PCR (viral loads, multiplex PCR, HPV, genetics)

Molecular biology in Tunis, Tunisia · Centre Urbain Nord

Detection and quantification of the DNA or RNA of infectious agents by real-time PCR: viral loads, respiratory, digestive and meningeal multiplex PCR, HPV with genotyping, tuberculosis, and detection of genetic mutations.

What is it?

Molecular biology identifies a microbe or a genetic abnormality directly from its genetic material, by chain amplification (real-time PCR). It is faster and more sensitive than culture for many infectious agents, and makes it possible to measure the quantity of virus in the blood (viral load). The partner laboratory of the Centre Médical Pasteur performs in particular: viral loads for hepatitis B and C, HIV, cytomegalovirus and Epstein-Barr virus; detection of infectious agents on all types of samples (Chlamydia, gonococcus, mycoplasmas, Helicobacter pylori, herpes, whooping cough, influenza, respiratory syncytial virus, tuberculosis with detection of resistance to anti-tuberculosis drugs); syndromic multiplex PCR panels that detect in a single analysis the main viruses and bacteria of a respiratory, digestive or meningeal infection or of a blood culture; human papillomavirus (HPV) screening with complete genotyping of high- and low-risk subtypes. It also offers the detection of genetic mutations useful in haematology and cardiology (factor V Leiden, factor II, MTHFR, JAK2, BCR-ABL) and HLA typing (DQ2/DQ8 for coeliac disease, B27, B51).

When is it indicated?

Hepatitis B or C, HIV: viral load

The quantity of virus in the blood measures the activity of the infection and the effectiveness of antiviral treatment.

Severe respiratory infection or fragile patient: multiplex PCR

A single analysis quickly identifies the virus or bacterium responsible and avoids unnecessary antibiotics.

Infectious diarrhoea, suspected meningitis

Digestive and meningeal multiplex PCR give a rapid answer in situations where every hour counts.

Cervical cancer screening: HPV

Detection and genotyping of papillomavirus on a cervical sample identify the subtypes at high risk of precancerous lesions.

Genital infections: Chlamydia, gonococcus, mycoplasmas

PCR detects these germs with great sensitivity, even without symptoms.

Recurrent thrombosis, coeliac disease, spondyloarthritis: genetic tests

Testing for factor V Leiden or HLA typing helps understand a predisposition and adapt prevention.

Preparation

1No fasting needed
2Take the sample before any antibiotic or antiviral treatment whenever possible
3HPV: cervical sample, away from menstruation, with no intimate hygiene or intercourse within 48 hours
4Respiratory or genital samples: follow the collection instructions given by the laboratory
5Bring the prescription and specify the clinical context, useful for interpretation

Procedure

1

Appropriate sample: blood, swab, urine, CSF, sputum or cervical sample

2

Extraction of the genetic material in a dedicated area

3

Amplification by real-time PCR on an automated system

4

Analysis of the curves and validation by the biologist

5

Report specifying the presence, quantity or genotype detected

Duration

Results within 24 hours for most PCR tests, longer for some genetic tests

Results

The report indicates whether the genetic material sought is detected, its quantity for viral loads, and the genotype for HPV. The biologist comments on the result and remains available to discuss its meaning with the doctor.

Risks and side effects

No risk linked to the analysis; possible brief discomfort during swabbing
A poorly taken sample can make the analysis uninterpretable and need to be repeated

These risks remain rare. Your physician will inform you in detail before the examination.
