Carpal tunnel surgery

Carpal tunnel in Tunis, Tunisia · Centre Urbain Nord

Release of the median nerve compressed at the wrist, to relieve tingling and numbness in the fingers.

What is it?

The carpal tunnel is a narrow passage at the front of the wrist. The median nerve, which provides feeling to the thumb, index and middle fingers, runs through it with the finger tendons. When the tunnel becomes too tight, the nerve is compressed: tingling, numbness and pain, often at night, followed by clumsiness and weakness of the hand. The operation consists of cutting the ligament that forms the "roof" of the tunnel to give the nerve more room. It is done through a small incision in the palm or, in some cases, endoscopically through an even shorter incision. The ligament then heals in a wider position. Surgery is offered when symptoms persist despite a night splint or a steroid injection, or straight away when the electromyogram (nerve test) shows significant nerve damage. The earlier the nerve is released, the better the recovery. At Clinique Pasteur Tunis, the electromyogram can be carried out in the neurophysiology department, and the operation is done as a day case under local anaesthesia or a nerve block of the arm.
Carpal tunnel surgery

When is it indicated?

Night-time tingling in the fingers

Numbness of the thumb, index and middle fingers that wakes you at night and makes you shake your hand.

Splint or injection no longer effective

When simple treatments are no longer enough or the symptoms keep coming back.

Weakness or clumsiness

Difficulty holding objects or doing up buttons, dropping things.

Nerve damage on the electromyogram

The nerve test confirms the compression and measures how severe it is.

Wasting of the muscle at the base of the thumb

A sign of long-standing compression; surgery should then not be delayed.

Preparation

1Electromyogram to confirm the diagnosis
2Consultation with the surgeon and, if needed, an anaesthetic consultation
3Tell the team about your medication, especially blood thinners
4Remove rings and bracelets from the hand to be operated on
5Arrange for someone to drive you: no driving on the day

Procedure

1

Admission to the day surgery unit

2

Local anaesthesia or a nerve block of the whole arm

3

Small incision in the palm, or endoscopic technique

4

Division of the ligament compressing the nerve

5

Closure with a few stitches and a dressing

6

Discharge a few hours after the operation

Duration

About 15 to 30 minutes

Aftercare & Recovery

You go home the same day. The fingers should be moved from the first day. Night-time tingling often disappears quickly; recovery of feeling and strength can take several months if the nerve was severely compressed. Tenderness in the palm may last a few weeks. Light activities resume within days, heavy manual work after a few weeks.

Risks and side effects

Pain or tenderness of the palm scar, which is temporary
Temporary swelling and stiffness of the hand
Complex regional pain syndrome, which is uncommon
Infection, haematoma or nerve injury, which are rare
Incomplete recovery if the compression was long-standing

These risks remain rare. Your surgeon will inform you in detail before the procedure.
