Diagnostic laparoscopy in gynaecology

Diagnostic laparoscopy in Tunis, Tunisia · Centre Urbain Nord

Examination of the pelvis with a small camera inserted through the navel, to understand pain or infertility and treat at the same time.

What is it?

Diagnostic laparoscopy allows the surgeon to look directly at the womb, fallopian tubes, ovaries and the lining of the abdomen (peritoneum). A small camera is inserted through an incision of a few millimetres in the navel after the abdomen has been inflated with gas. One or two other small incisions allow fine instruments to be passed. During the examination, the surgeon can check whether the tubes are open by injecting a coloured fluid through the cervix (dye test), look for endometriosis or adhesions, and take samples. In most cases, what is found can be treated under the same anaesthetic: releasing adhesions, treating small areas of endometriosis, removing a cyst. This procedure is offered when imaging does not explain persistent pelvic pain, in some infertility assessments, or when a lesion in the pelvis needs to be seen directly. The decision is always discussed with the patient, weighing up the expected benefit. At Clinique Pasteur Tunis, laparoscopy is performed in the operating theatre with a laparoscopy stack, by gynaecologists experienced in the technique. The surgeon explains to the patient what was seen and done, on waking or the following day.

When is it indicated?

Persistent unexplained pelvic pain

When the lower abdomen has been painful for a long time and no cause is found on ultrasound or MRI, looking directly often reveals the source.

Infertility assessment

Laparoscopy checks the condition of the tubes and looks for adhesions or endometriosis that could interfere with pregnancy.

Suspected endometriosis

It confirms the disease, shows how extensive it is and, often, allows the lesions to be treated at the same time.

Adhesions after an infection or surgery

Tissues stuck together can cause pain or block the tubes. They can be released during the operation.

Pelvic mass needing clarification

When an image remains unclear despite tests, laparoscopy allows it to be seen, sampled or removed.

Preparation

1Pelvic ultrasound, and MRI if requested
2Anaesthetic consultation and blood tests
3Operation scheduled outside the period if possible
4Fasting for 6 hours before the operation
5Mention all current treatments, especially blood thinners

Procedure

1

Admission on the day of the operation

2

General anaesthetic

3

Small incision in the navel and inflation of the abdomen with gas

4

Insertion of the camera and one or two fine instruments

5

Examination of the womb, tubes, ovaries and peritoneum, with a dye test if needed

6

Treatment of any lesions found when possible, and samples taken

7

Release of the gas, closure of the small incisions and monitoring in the recovery room

Duration

30 minutes to 1 hour 30 depending on the procedures performed

Aftercare & Recovery

Patients go home the same day or the next day. Pain is moderate; shoulder discomfort caused by the gas may last one or two days. Activities can be resumed within about a week. The surgeon explains what was seen and suggests the next steps: monitoring, medical treatment, further surgery or fertility care.

Risks and side effects

Temporary shoulder pain and bloating caused by the gas
Small bruise or infection of an incision, usually minor
Injury to a nearby organ (bowel, bladder, blood vessel), rare
Conversion to a larger incision if the situation requires it, exceptional

These risks remain rare. Your surgeon will inform you in detail before the procedure.
