Operative hysteroscopy (polyp, septum, fibroid)

Operative hysteroscopy in Tunis, Tunisia · Centre Urbain Nord

Treatment of polyps, septa, small fibroids and adhesions inside the womb through the natural passages, with no incision.

What is it?

Operative hysteroscopy treats abnormalities inside the womb through the natural passages. A hysteroscope slightly wider than the diagnostic one is passed through the vagina and cervix; it carries fine instruments used to remove or divide the lesion under direct vision. There is no scar on the abdomen. The most common procedures are removal of a polyp from the lining, removal of a fibroid that bulges into the cavity (submucosal fibroid), division of a uterine septum and release of adhesions. Any tissue removed is always sent for analysis. The operation takes place in the operating theatre, under a short general anaesthetic or a spinal anaesthetic. These lesions can cause heavy periods, bleeding between periods or after the menopause, difficulty conceiving or miscarriage. Treating them by hysteroscopy preserves the womb and the possibility of pregnancy, with a quick recovery. At Clinique Pasteur Tunis, operative hysteroscopy is carried out as a day-case procedure in a modern operating theatre. Before the operation, the gynaecologist explains what is planned; afterwards, what was done and the recommended follow-up.

When is it indicated?

Endometrial polyp

This is a small growth of the lining of the womb. It can cause bleeding or interfere with pregnancy; it is removed and analysed.

Fibroid inside the cavity (submucosal)

A fibroid that protrudes into the womb often causes heavy periods. If it is accessible, it can be removed without opening the abdomen.

Uterine septum

Some women are born with a wall of tissue dividing the womb. Dividing it can reduce the risk of miscarriage.

Adhesions inside the womb (synechiae)

After a curettage or an infection, the walls of the womb can stick together. Releasing them helps restore normal periods and fertility.

Removal of a coil that is difficult to remove

When the threads of a coil can no longer be seen or the coil has moved, hysteroscopy allows it to be removed under direct vision.

Aftercare & Recovery

Most patients go home the same day. Light bleeding and cramps may last a few days. Activities can be resumed within one to three days. It is advisable to avoid baths, tampons and intercourse for about a week. A follow-up appointment discusses the analysis; a check hysteroscopy is sometimes offered, especially after division of a septum or adhesions.

Risks and side effects

Light bleeding and cramps for a few days
Infection of the womb, uncommon and treated with antibiotics
Perforation of the wall of the womb, uncommon and usually without consequences
Excess absorption of fluid into the bloodstream, monitored throughout the procedure
A polyp may recur or adhesions may reform, hence the follow-up

These risks remain rare. Your surgeon will inform you in detail before the procedure.
