Scar revision (unsightly scar, keloid)

Scar revision in Tunis, Tunisia · Centre Urbain Nord

Improvement of a wide, thick, contracted or keloid scar through surgical revision and associated treatments.

What is it?

A scar can become troublesome: too wide, raised, red, contracted to the point of limiting movement, or poorly placed. A keloid is a particular form: the scar keeps growing beyond the original wound, itches and can recur. It is more common on darker skin and in certain areas such as the chest, shoulders or ears. Scar revision most often involves removing the damaged scar and closing the skin with a fine suture, without tension. When a scar pulls on a joint or the face, plasty techniques (such as a Z-plasty) can change its direction and release the contracture. A scar never disappears: the aim is to make it finer, flatter and less visible. For keloids, surgery alone often leads to recurrence. It is therefore combined with other treatments: corticosteroid injections into the scar, pressure dressings, silicone sheets and sometimes radiotherapy. Before operating again, a recent scar must also be given time to mature, often a year or more. These points are explained in consultation. At Clinique Pasteur Tunis, scar revision is performed by a plastic surgeon, often under local anaesthesia and as a day case, with prolonged monitoring of healing.
Scar revision (unsightly scar, keloid)

When is it indicated?

Wide, thick or raised scar

A scar that has widened or thickened can be revised to obtain a finer, more discreet line.

Keloid scar

A scar that spreads beyond its borders, itches and grows needs combined treatment to limit recurrence.

Contracted scar that limits movement

A scar band on a finger, elbow or the neck can hinder movement; a plasty releases the skin.

Poorly placed or visible scar on the face

A facial scar that does not follow the natural lines can be repositioned so that it blends in better.

Preparation

1Consultation with examination of the scar and photographs
2Waiting for the scar to mature (often a year) before operating again, unless a troublesome scar band is present
3Mentioning any personal or family history of keloids
4Anaesthesia consultation if general anaesthesia is planned
5Stopping smoking recommended

Procedure

1

Local anaesthesia in most cases, general for large areas

2

Removal of the damaged scar

3

Plasty (Z-plasty or other) if the scar needs to change direction

4

Fine suturing in several layers, without tension

5

For a keloid: corticosteroid injection at the end of the procedure depending on the case

6

Dressing and care instructions

Duration

30 minutes to 1.5 hours depending on the scar

Aftercare & Recovery

Discharge is on the same day in most cases. Stitches are removed after one to two weeks depending on the area. The new scar must be protected from the sun and massaged as advised by the surgeon. It continues to change for a year or more. For a keloid, close follow-up and additional treatments are often needed.

Risks and side effects

New scar that widens or thickens
Recurrence of the keloid, hence the associated treatments
Infection or wound breakdown, rare
Change in skin colour around the scar

These risks remain rare. Your surgeon will inform you in detail before the procedure.
